I've got a 2001 Td5 defender with 145k. It's just developed a serious smoking issue. The smoke is only present at idle and is a blue/white colour, also it seems to require topping up with water after 2 days.
Am I right in thinking this is a cracked head??

White smoke which smells of diesel usually means too much air/not enough diesel.
White smoke which does not smell of diesel or smells sweet and cloying is usually coolant leaking into cylinders. It could be a cracked head, or it could be head gasket.
I'd try a coolant pressure test.
